Required Medical Records

Safety is our number one priority at All About My Dog. Ensuring a safe and controlled environment allows our students to learn, thrive, and achieve their highest potential. Therefore, the following requirement is strictly enforced:

Up-to-date medical records, including a negative fecal report dated within one year that states “no parasites seen,” must be submitted to us at least 72 hours prior to your first session. You can submit your dog’s records below(There are no refunds or make-ups for appointments forfeited due to missing or late fecal records.) 

Fecal Lab Results

To participate in any training session, an up-to-date fecal report must be on file at AAMD. This report must be dated within one year of your training session and must state “No Parasites Seen.” When your dog is tested, remember to ask your vet for a hard copy of the results, or have them email the results to us directly. (See How To Submit Your Dog’s Records below.)

Medical Leave

Positive for Giardia or other parasites

If your dog tests positive for giardia or other parasites, please alert All About My Dog immediately. Any dog that tests positive will be immediately placed on medical leave. Dogs who have tested positive for parasites may not begin or return to training until a new, negative fecal test is submitted. Please follow your vet’s prescribed treatment (usually 2 weeks), then request a new fecal test for confirmation. Email the results to [email protected] or submit them using the form below. Once we verify the results, you can resume training. Contact your trainer to reschedule your session.

Other medical issues

Let us know if your dog is ill or injured and your vet has recommended limited activity or no contact with other dogs. If your dog is scheduled for training, you may be eligible for a credit that can be used once your vet clears your dog for activity. Be sure to contact your trainer to arrange your return to training.

Vaccinations

All dogs six months and older must be vaccinated for Rabies as required by Massachusetts state law. All other vaccinations must be current per the recommendation of your veterinarian.

How To Submit Your Dog’s Records

Records must be submitted prior to your first training session. Remember that the records must include a negative fecal report stating “No Parasites Seen.” Several options for submitting your dog’s records include:

  1. Email an electronic version of your dog’s records to [email protected]. (pdf files are recommended.)
  2. Email an electronic version of your dog’s records to your specific trainer. Trainer emails are available here. (pdf files are recommended.)
  3. Ask your veterinarian to forward your dog’s records to [email protected].
  4. Mail a hard copy of your dog’s records to All About My Dog, 42 Worcester St., Natick, MA 01760.
  5. Upload an electronic version of your dog’s records using the form below. (pdf files are recommended.)
